When Elijah Johnson, a 34-year-old Black man from Chicago, sought help for persistent feelings of exhaustion, irritability, and difficulty concentrating, the AI-powered assessment tool at his local clinic categorized his symptoms as ‘low-risk’ for clinical depression, a stark example of AI bias in mental health care. Six months later, after his condition worsened, a human psychiatrist diagnosed him with major depressive disorder that had gone untreated for months, raising concerns about how algorithmic biases may overlook or misdiagnose symptoms in marginalized communities.
“The system didn’t recognize how depression manifests differently in my community,” Johnson explained. “I kept being told I was just stressed when I knew something deeper was wrong.”
Johnson’s experience isn’t isolated. As artificial intelligence increasingly infiltrates mental health care systems across America, a troubling pattern has emerged: the very algorithms designed to improve care accessibility are systematically failing patients from marginalized communities, particularly people of color.
AI systems have rapidly transformed mental health services over the past five years. From initial screening tools to treatment recommendation engines and even therapeutic chatbots, these technologies promised to democratize access to mental health support. According to the American Psychological Association’s 2025 Technology Survey, approximately 78% of mental health facilities now utilize some form of AI in their assessment or treatment protocols.
However, a growing body of research reveals that these systems perpetuate and sometimes amplify existing disparities. The National Institute of Mental Health’s comprehensive 2024 report found that AI diagnostic tools misclassify symptoms in Black patients at rates 2.7 times higher than in white patients. For Latino and Asian American patients, misclassification rates were 1.9 and 1.6 times higher, respectively.

Stanford University’s AI Ethics Initiative analyzed the five most commonly used mental health datasets powering commercial AI systems and found alarming demographic imbalances:
“This is garbage-in, garbage-out at scale,” explains Dr. James Liu, computational psychiatrist at MIT’s Medical AI Lab. “When these systems are deployed nationwide but trained on narrow population slices, the technology inherently becomes discriminatory.”
Recent attempts to diversify training data have produced modest improvements but continue to fall short of true representational equity. The Mental Health Equity in AI Consortium, formed in late 2024, aims to create more inclusive datasets but acknowledges this will take years to fully implement.

Community mental health advocates have been sounding alarms about algorithmic discrimination for years, often before formal studies confirmed their observations.
“We noticed immediately that patients who described their symptoms using culturally specific language were being flagged as ‘inconsistent’ by these systems,” says Teresa Gonzalez, director of La Esperanza Mental Health Collective in Los Angeles. “Our community doesn’t always use words like ‘depressed’ or ‘anxious’ – they might talk about a heaviness in the heart or a restless spirit. The AI doesn’t understand these expressions.”
Darryl Washington, who leads the Black Mental Health Alliance in Baltimore, describes a particularly troubling pattern: “Black men expressing anger or frustration as symptoms of depression are routinely misclassified as having conduct or personality disorders instead. The algorithms seem to carry the same biases as the society that created them.”
The FDA’s landmark AI in Healthcare Equity Act of 2024 established the first federal requirements for demographic testing of medical AI systems. Under these regulations, developers must demonstrate that their algorithms perform consistently across different racial, ethnic, and socioeconomic groups before receiving approval.
However, implementation has proven challenging. Of the 37 mental health AI systems reviewed under the new standards, only 8 have received full approval, with the remainder operating under provisional licenses while addressing equity concerns.
Dr. Elena Rodríguez, commissioner of the FDA’s Digital Health Division, acknowledges the challenges: “We’re seeing resistance from some companies who claim that demographic parity is technically impossible to achieve. Our position is clear: if your system can’t serve all Americans equitably, it shouldn’t be in clinical use.”
Leading AI mental health companies have responded variably to these pressures. Some, like MindTech Solutions, have invested substantially in retraining their systems with more diverse datasets. Others, including industry giant PsychAI, have opted to add disclaimers about potential limitations when used with certain populations – a response many advocates find inadequate.
“Adding a footnote that your product might not work for Black or Latino patients doesn’t solve the problem – it institutionalizes it,” argues civil rights attorney Jasmine Reynolds.
Despite these challenges, innovative approaches to addressing algorithmic bias in mental health care are emerging:
The Justice in Healthcare AI Coalition, comprising over 200 organizations including the NAACP and National Council of La Raza, has published a comprehensive framework for equity in mental health algorithms. Their report, released in March 2025, established measurable standards for demographic parity and called for mandatory transparency in training data composition.
AI holds tremendous potential to extend mental health care to underserved populations – but only if these systems work equitably for all communities. As Dr. Richardson from the Center for Responsible AI in Healthcare notes, “The issue isn’t whether we should use AI in mental health care, but rather how we can ensure these powerful tools serve everyone fairly.”
For patients like Elijah Johnson, the stakes couldn’t be higher. “Technology is supposed to make things better, not worse,” he reflects. “But when the computer doesn’t understand how depression looks on someone who looks like me, that’s not progress – it’s just finding a new way to leave people behind.”
As AI continues transforming mental health services, the industry faces a critical inflection point: Will developers and providers commit to the challenging work of building truly inclusive systems, or will algorithmic bias become another barrier to equitable care? The answer will shape mental health outcomes for millions of Americans from marginalized communities for decades to come.
